Camp Middlesex 2026 Health & Waiver Form

Please complete the Camp Middlesex Health and Waiver Form for each camp participant.  Registrations cannot be processed until this completed form is reviewed and approved by our staff. This process may take up to 48 hours, and you will receive an email once your application is approved or if our staff have any questions regarding your information.
